RULES AND REGULATIONS – ELIGIBILITY: Entry purchaser, and anyone to whom an entry purchaser legally transfers an entry, must be either a business entity or an individual 21 years or older and a legal U.S. resident at time of entry purchase. It is not necessary to be a resident of the state of Arizona, but one must be physically present in Arizona at the time of entry purchase. Tickets will be sold through Ronald McDonald House Charities of Phoenix (RMHC) Board Members, authorized RMHC employees and authorized RMHC volunteers. Tickets may be purchased by phone at (602) 798-5097, online at www.mcnightgala.com, or onsite at 501 E. Roanoke Ave., Phoenix, AZ 85004 (RMHC Front Office). By law, raffle and table reservations must be purchased with a separate check or charged to a credit card. Due to postal regulations, raffle ticket stubs cannot be returned by mail should tickets be purchased online or over the phone. Accordingly, an authorized representative from RMHC shall complete the ticket by filling in the name, address, phone number and e-mail address of purchaser. It is the responsibility of ticket purchaser to correctly provide contact information including name, address, telephone number and e-mail
address so that they can be promptly contacted should they be deemed a winner. A receipt of purchase with ticket numbers will be mailed to ticket purchaser. DRAWING: Only 2,000 tickets will be sold at two price points: single tickets for $20 and 6 for $100. RMHC assumes no responsibility for lost, late, misdirected, mutilated, incomplete, illegible or non-delivered entries. Any mutilated, incomplete or illegible entries will be deemed invalid and shall be immediately disqualified. All entries received by October 18 will qualify for a chance to win the early bird raffle prize of two (2) round-trip Southwest Airlines tickets. There will be one (1) early bird raffle winner to be announced on October 19, 2011. All entries will be entered in the main drawing to be held at the gala on November 5, 2011. One winning entry will be drawn at the McNight to Remember Gala on November 5, 2011 at the Sheraton Phoenix Downtown Hotel. Winner need
not be present to win. Winner not present at time of drawing will be notified by phone and/or mail no later than November 7, 2011. Winner will select one (1) of the three (3) prizes offered. Once a prize is selected, winner will be mailed prize redemption instructions and certificates. Please allow 3-4 weeks to receive your prize package from time of prize selection. Once a prize selection has been confirmed there will be no returns or exceptions. ADDITIONAL INFO: All airfare certificates will have a one-year expiration date from purchase. Any airport taxes, fees, or fuel surcharges (if charged) are the responsibility of the winner. Prize must be booked a minimum of 60 days in advance of trip date. Reservations are subject to availability. Certificates cannot be replaced if lost, stolen or destroyed. All purchases are non-refundable. Packages cannot be resold. Suggested retail value is calculated using tariff air rates and rack room rates. Ground transportation is not included unless where specified. TAX & IRS RULES: All proceeds benefit Ronald McDonald House Charities of Phoenix, a non-profit organization operating under paragraph 501(c)(3) of the Internal Revenue Code and the laws of the State of Arizona. Winner shall be responsible for any applicable federal, state, and local taxes. The IRS has taken the position that amounts paid for chances to participate in raffles, lotteries or similar programs are not gifts and, therefore, the price of the entry does not qualify as a deductible charitable contribution or donation for income tax purposes.
